Physician & Nurse Practitioner Services

Every patient at Anchor of Love Hospice is under the care of a hospice physician working alongside our nurse practitioner. Together they establish the plan of care, certify eligibility, and review the treatment approach as the illness progresses.

You are not asked to give up your own doctor. Patients may keep their attending physician, who works with our medical team rather than being replaced by it.

What this includes

• Initial assessment and certification of hospice eligibility
• An individualized plan of care, reviewed regularly
• Face-to-face visits as the illness progresses
• Coordination with your existing attending physician
• Prescribing and adjusting medication for comfort
