Care Coordination & Medical Director Oversight

Hospice involves a lot of people — physician, nurse practitioner, nurses, aides, therapists, social workers, chaplains and volunteers. Care coordination is what stops that becoming a burden on the family.

The interdisciplinary team meets regularly to review each patient, and our medical director provides clinical oversight of every plan of care. You should never have to relay information between members of your own care team.
