Hospice is a type of care for people with a serious illness who are no longer seeking curative treatment. Rather than trying to cure the illness, hospice focuses on relieving symptoms and supporting quality of life. Care is delivered by an interdisciplinary team and, in most cases, takes place wherever the patient calls home.
A patient is generally eligible when a physician certifies that they have a life-limiting illness with a prognosis of six months or less if the illness runs its expected course, and the patient chooses comfort-focused care rather than curative treatment. Eligibility is re-certified periodically, and patients who improve can be discharged from hospice.
Most hospice care is provided at home — a private residence, an assisted living facility, or a nursing home. Our team comes to the patient.
A physician and medical director, nurse practitioner, registered nurses, licensed vocational nurses, certified nursing assistants, physical therapists, social workers, chaplains and trained volunteers — coordinated around one individualized plan of care.
Care related to the terminal diagnosis: visits from the team, pain and symptom management, medications, durable medical equipment such as a hospital bed, oxygen or a wheelchair, incontinence and other supplies, plus emotional, spiritual and bereavement support for the family.
No. It means shifting the goal of care from cure to comfort. Many families tell us their loved one was more comfortable, and more present with them, after starting hospice. Patients may also leave hospice at any time if they decide to pursue curative treatment again.
