Anyone can start the conversation — a patient, a family member, a physician, or a hospital discharge planner. A physician certification is required for hospice admission, and we coordinate that as part of the evaluation.
Yes. We provide 24/7 on-call nursing support. If something changes at 3am, there is a nurse to call.
Yes. Patients may keep their attending physician, who works alongside our medical director and care team.
